Employees have the option to choose one of two medical plans that best fit your family. One plan is offered through Surest. The Surest plan offered does not have a deductible associated with its plan design. It is an innovative copay-based plan that simplifies the member billing experience and encourages members to use online resources to research quality providers and the cost of tests, procedures, and office visits. The other plan offered is through UnitedHealthcare and is a Health Savings Account Qualified High Deductible Health Plan. Both medical plans utilize UnitedHealthcare's Choice Plus national network.​

Dental
Access your UHC dental benefits and download your ID card anytime by registering online at www.myuhc.com or by downloading their mobile app.

Vision
ID cards are not required for your UHC vision benefits. Providers can call UHC directly at 800-638-3120 to confirm benefits and eligibility. Employees can access benefit information online by registering at www.myuhcvision.com.

Life and Disability
Basic Life AD&D, Short-Term Disability, and Long-Term Disability benefits are provided 100% by Lockstep Technology Group. Employees can purchase additional life insurance under the Voluntary Life benefit, and this is paid 100% by you. Employees can access benefit information online by registering at www.myuhcfp.com.

Accident, Critical Illness, and Hospital Indemnity Insurance
Employees can purchase Accident, Critical Illness, and Hospital Indemnity insurance to help reimburse you for out-of-pocket expenses under your medical plan. These coverages are paid 100% by you.
